Hip Replacement Surgery

During hip replacement surgery, a surgeon removes the damaged sections of the hip joint and replaces them with prosthetic components typically made of metal, ceramic, and very hard plastic. This artificial joint (prosthesis) helps reduce pain and improve function.

Also known as total hip arthroplasty, hip replacement surgery may be considered if hip pain significantly interferes with daily activities and nonsurgical treatments have failed to provide relief or are no longer effective. The most common reason for needing hip replacement is damage from arthritis.

The Reason Why it's necessary

Hip replacement surgery may become necessary due to conditions that damage the hip joint, including:

  • Osteoarthritis: Commonly known as wear-and-tear arthritis, osteoarthritis damages the slick cartilage that covers the ends of bones and helps joints move smoothly.

  • Rheumatoid Arthritis: This autoimmune condition causes inflammation that can erode cartilage and sometimes the underlying bones, resulting in damaged and deformed joints.

  • Osteonecrosis: Insufficient blood supply to the ball portion of the hip joint, which can occur following a dislocation or fracture, may cause the bone to collapse and deform.

Who should consider this surgery

  • Persists despite the use of pain medication,

  • Worsens with walking, even when using a cane or walker,

  • Interferes with sleep,

  • Affects the ability to navigate stairs,

  • Makes it difficult to rise from a seated position.

Pre-surgical Assessment and Preparation

Before the operation, you will undergo a thorough examination with your orthopedic surgeon. During this pre-surgical assessment:

  • Medical History Review: The surgeon will inquire about your medical history and current medications.

  • Physical Examination: Your hip will be examined to assess the range of motion and strength of the surrounding muscles.

  • Diagnostic Tests: You may undergo blood tests and an X-ray. An MRI is rarely necessary.

During this appointment, it is crucial to ask any questions you have about the procedure and clarify which medications you should avoid or continue taking in the week before surgery. Additionally, if you use tobacco, it is advisable to quit as it can interfere with the healing process. If needed, seek assistance from your doctor to stop using tobacco products.

Day of Surgery

  • Preparation: Upon checking in for your surgery, you will be asked to remove your clothes and put on a hospital, gown.

  • Anesthesia: You will receive either a spinal block, numbing the lower half of your body, or a general anesthetic, which induces a sleep-like state.

  • Pain Management: Your surgeon may inject a numbing medicine around nerves or in and around the joint to help control pain after the surgery.

During the Procedure

The surgical procedure typically lasts about two hours:

  • Incision: The surgeon makes an incision over the hip, through the layers of tissue.

  • Removal of Damaged Joint: Diseased and damaged bone and cartilage are removed, leaving healthy bone intact.

  • Implant Placement: The surgeon implants the replacement socket into the pelvic bone and inserts a metal stem topped with a replacement ball into the thighbone.

After the procedure

After the Surgery

  • Recovery Area: You will be moved to a recovery area where your anesthesia will wear off over a few hours. Medical staff will monitor your vital signs, such as blood pressure and pulse, as well as your alertness, pain levels, and medication needs.

  • Postoperative Care: You will be encouraged to breathe deeply, cough, or blow into a device to help keep fluid out of your lungs.

After your knee replacement surgery at Ratchaphruek Hospital, you are required to stay in the hospital. During this stay, you will have access to a range of specialized facilities and services that ensure your comfort and safety. Please note that these are not included in the surgery package. Here are the key services including the hospital stay:

  • Nurse on Call: 24-hour medical assistance.

  • Safety Bathroom: Equipped with necessary aids to ensure safety.

  • Adjustable Bed: Helps in adjusting your position comfortably.

  • Bed for Family Member: Provides a space for a family member to stays close.

  • Customized Food and Menu: Nutritious meals tailored to your health needs.

  • Rehabilitation Program: On-site physical therapy sessions to aid recovery.

  • In-Room Follow-Up Care: Frequent monitoring by medical staff.

Our hospital offers these services at considerate rates, often more favorable than what would be found in a hotel, providing both medical care and accommodation. This approach allows us to focus on your recovery in a supportive and comprehensive manner.

Physical Therapy

Daily activity and exercise are crucial for regaining the use of your joint and muscles after hip replacement. A physical therapist will guide you through strengthening and mobility exercises and teach you how to use walking aids like walkers, canes, or crutches. As your therapy progresses, you will gradually increase the weight you place on your leg until you can walk without assistance.

Home Recovery

To ensure a smooth transition from the hospital to home recovery, consider the following:

  • Meal Preparation: Arrange for a friend or relative to prepare meals in advance.

  • Accessibility: Place everyday items at waist level to avoid bending down or reaching up.

  • Safety Modifications: Consider installing a raised toilet seat and a shower chair to enhance safety and comfort in your bathroom.

  • Convenience: Keep essentials like your phone, tissues, TV remote, medications, and books within easy reach of where you will spend most of your recovery time.

Results

Recovery times can vary, but most people significantly improve within three months after surgery. Improvements often continue for the first year post-operation. While the new hip joint can reduce pain and enhance mobility, it may not allow you to engage in the high-impact activities you participated in before your hip became painful.

Post-Recovery Activities

  • High-impact activities such as running or playing basketball may place too much stress on the artificial joint.

  • Lower-impact activities like swimming, golfing, and cycling are usually more suitable and can be enjoyed as you recover further.
